Marx and Weberââ¬â¢s Analyses of the Development of Capitalism Essay Capitalism is defined as ââ¬ËAn economic and political system in which a countrys trade and industry are controlled by private owners for profit.ââ¬â¢ It is based on the division between two classes, one of which owns the labour of the other. Not only do the upper classes, or the bourgeoisie, own the means of physical production but also the means of ââ¬Ëmental productionââ¬â¢. They control and manipulate society through the rule of education, religion and the media. Althusser distinguishes between repressive state apparatuses and ideological state apparatuses and argues about how the bourgeoisie manages to maintain its rule. He argues that the repressive includes the police and the army in which use physical force to control the working class as opposed to the ideological apparatuses such as the media and religion which control the development of ideas. A key component of capitalism is that the working class are forced to sell their labour in exchange for wages in order to survive. However, they do not receive an equal exchange for the labour they produce, but only the cost of subsistence. The difference of what the bourgeoisie receive from the labourers and the amount they pay back is called the surplus value, meaning the profit they make. Max Weber was one of the founding fathers of sociology and contributed highly to our knowledge of how society works. Weberââ¬â¢s work can be highlighted by referring to his study The Protestant Ethic and the Spirit of Capitalism, first published in 1905 (22 years after the death of Karl Marx in 1883). Weber argues that the Protestant Reformation introduced a new belief system of Calvinism (a form of Protestantism founded by John Calvin during the reformation) which promoted a high work ethic and which eventually led to the rise in capitalism. Calvinists believed that God preordains the ââ¬Ëelectââ¬â¢ meaning of who would be saved after death and go onto heaven and who would not. This could not be changed through hard work or leading a good life as the decision had already been decided. This made Calvinists strive for success, with which they would reinvest into making more money, hence the development of capitalism. Weber distinguishes the differences between the capitalism of greed and wealth in past societies to those of present. Modern day people are pursing profit for its own sake rather than for consumption, hence why the Calvinists reinvested their wealth. Weber calls this the spirit of capitalism. He further argues that this was the reason capitalism was stronger in places like Europe and America and not in other places where Protestantism wasnt so established. Weber also distinguishes between many different existing forms of capitalism including ââ¬Ëtraditional capitalismââ¬â¢ and ââ¬Ëbooty capitalismââ¬â¢; however the crucial ideal type is the one named modern capitalism, or rational capitalism meaning the repetitive, ongoing economic activity on the basis of rational calculation. Understanding what needs to happen and what the best way of achieving it is, allows for reinvestment and the growth of economic enterprises. He argues that it is the rational side of modern capitalism that distinguishes it from other advanced economic areas such as China and India, both of which had higher and more advanced infrastructures in the 17th century compared to Europe and America. However, Weber is hugely criticised for his understanding of the rise in capitalism due to others believing that it was the peoples relationship with the material forces and there means of subsidence which drove the change. Weber takes a key focus on religion and the impact that had on the rest of society as well as capitalism, whereas Marx focuses on class conflict. Marx argues that through industrialisation capitalism had been forced to increase due to growing separation of the two contrasting classes. One class is the exploiting bourgeoisie who own the means of production and the other class being the proletariat who own nothing but their own labour. Marx predicted that the working class would eventually become conscious of their alienation and exploitation and unite to overthrow capitalism. This would slowly bring in a system of socialism which would gradually evolve into a pure classless communist society lacking in exploitation. He argued that capitalism would disintegrate due to interior tensions, just like every other social system. He believed that communism was inevitably the next stage in the line of historical changes to class systems. Just as feudalism was replaced by capitalism, so capital ism would be replaced by communism. Marx argues that religion performs a different function than that of what Weber argues. Instead it operates as an ââ¬Ëideological weaponââ¬â¢ used by the bourgeoisie to justify the suffering of the poor as something unchangeable and ââ¬Ëgod-givenââ¬â¢. Religion persuades the working class that their suffering is honourable and moral and will be favoured in the afterlife. This is evident in the Christianity teaching of it is ââ¬Ëeasier for a camel to pass through the eye of a needle than it is for a rich man to enter the kingdom of heavenââ¬â¢. This manipulates and oppresses the proletariat as it renders them blind to capitalistic trends; ensuing and maintaining false class consciousness. However, Marx can be criticised for ignoring the positive functions that religions perform, made apparent by the psychological adjustment to misfortune that it offers. Abercrombie and Turner (1978) argue that ââ¬Ëin pre capitalist society, while Christianity was a major element of ruling-class ideology, it had only limited impact on the peasantryââ¬â¢ (A2 Sociology AQA Specification, 2009, pg 13) However, although Marx does argue that religion helps to control the manipulation of ideas of the working class he also believes that it is ââ¬Ëthe heart of the heartless world and the soul of soulless conditionsââ¬â¢, as it can act as an distraction to dull the pain of exploitation. When comparing and contrasting two very highly influential historians such as Karl Marx and Max Weber, some would argue that it is highly important to look at their overall impact on society as well as humanity. Karl Marx focused highly on philosophy and his work is still influential in many cultures worldwide today. This contrasts to Max Weber who is considered ââ¬Ëone of the fathers of modern thoughtââ¬â¢ and could be considered one of the worldââ¬â¢s most intellectual and influential persons. Although both historians share clear similarities, for example both coming from a European Protestant background they also contrast and have distinct differences. Weber criticises Marxââ¬â¢s theory as he believes that his view is too one dimensional and simplistic when looking at inequality. Weber argues that this is due to Marx seeing class as the only important division. Weber argues that status and power also have high impact on the volume of inequality. He points towards the ââ¬Ëpower eliteââ¬â¢ for evidence and argues that they can rule without actually owning the means of production. Currently there are many independent companies that can control and rule particular labourers without being a part of the bourgeoisie, it is not as simple as Marx likes to preach. A great amount of people are in other situations than the time when Marx was writing, for example ââ¬Ëdealers in information, managers and civil servantsââ¬â¢, meaning that the relative importance of the struggle between owners and workers has relatively declined. Although Marx and Weber have severe differences in their evaluation of modern capitalism their augments also share many similarities. They both believe that the economic system is a place where ââ¬Å"individuals are directed by abstractionsâ⬠(Marx). We must also take into account the times of which both sociologists were writing. Weber is writing nearly half a century later and focuses highly on the impact of power, wealth and prestige. He argues that these were the three main factors contributing to capitalism and the distinction of classes. This contrasts to Marx who focuses singularly on the impact of class and how the contrast of bourgeoisie and proletariat impacted on the rise of capitalism. However, both of their summaries of overthrowing capitalism share many similarities. Both sociologists argue that in order for capitalism to be overthrown the working class must unite together to overthrow the ruling class and free themselves from capitalist oppression. Case Study: Knee Injury Treatment Short case summary: 25 years old, female injured her right knee during basketball game. PCP evaluated her, gave her non-steroidal anti-inflammatory with P.T referral. Patient came 2 days after injury, with knee swelling and locking. . Patient pain and mechanism of injury are consistent with a medial meniscus injury. Diagnostic test: McMurrays test, was Positive with palpable click. Reliability of McMurrays test: Evans ET. al1 demonstrated a low level of agreement between the two examiners with inter-tester agreements ranging from poor for reproduction of a medial sensation (Kappa = à ¢Ãâ ââ¬â¢0.10) to fair (K = +0.38) for lateral pain. Validity of McMurrays test: shortage of statistics in the literature increased the risk that the positive test criteria can change the test outcome, irrespective of whether the test was performed in the same manner on the same patient.1 I will change my first choice. McMurrays test alone is weak diagnostic tool for medial meniscus injury, the review2 has demonstrated that the inter-tester reliability and sensitivity (sensitivity ranged from 27% to 70%, specificity figures (29-96%) of the McMurrays test is relatively low. Another reading meta-analysis3,supported to use joint line tenderness test, McMurrays test, and Apleys test. I will add the other two tests to get strong diagnostic evidence for medial meniscus injury. Three special tests-McMurrays, joint line tenderness (JLT), and Apleys were included in the meta-analysis. Sensitivity of McMurrays test is 70.5 (95% CI: 67.4 to 73.4) and its specificity of 71.1 (95% CI: 69.3 to 72.9). Joint line tenderness sensitivity of 63.3 (95% CI: 60.9 to 65.7) and its specificity of 77.4 (95% CI: 75.6 to 79.1). Sensitivity of Apleys test is 60.7 (95% CI: 55.7 to 65.5) and its specificity of 70.2 (95% CI: 68.0 to 72.4). Another reading4 supported Thessaly Test at 5 and 20 degrees (Evidence obtained from high quality randomized controlled trials, prospective studies, or diagnostic studies). Intervention: therapeutic exercises to restore muscular strength and aerobic fitness. I found two evidences for therapeutic exercise intervention. I think both are strong evidences. First one4: The supervised exercise group was significantly better than the home-based group regarding Sports Activity Rating scale and hop tests (Evidence level B).the same guidelines recommended that, Clinicians should consider a clinic-based exercise program in patients following arthroscopic meniscectomy to increase quadriceps strength and functional performance(Evidence level B). The second evidence5was meta-analysis and systematic review reported that: No studies described the effectiveness of exercise therapy compared to no exercise therapy in non-surgical patients with a meniscal lesion. I will not change my previous decision about therapeutic exercise as the main choice intervention for two reasons: First, the clinical guidelines support that choice with level B evidence. Second reason: although the second study is systematic review, meta-analysis study It didnt introduce a strong alternative to my choice. Outcome measure: lower extremity function scale (LEFS SCALE): The test evaluate the impairment of a patient with lower extremity musculoskeletal condition or disorders. Test measures initial function, progress of function, and outcome to design functional goals. In my case I use the LEFS for medial meniscus injury outcomes. Questionnaire is asked about 4 level of performance in 20 task questions that patient perform in daily life. Minimal score is 0(complete disability) and maximum score is 80(complete functional level) Minimal Detectable Change (MDC)6:Various Lower Extremity Injuries (medial meniscus injury): MDC= 9 points. Minimally Clinically Important Difference (MCID)6: Various Lower Extremity Injuries: MCID = 9 points. According to, Binkley ET al6. The LEFS isvalid compared to the SF-36 in target population, and reliable. The LEFS Sensitivity to outcome change was higher than the SF-36 in this population. The LEFS is applicable for clinical situations for individual patients and research. LEFS SCALE reliability: Test-retest Reliability, Various Lower Extremity Injuries: Excellent test-retest reliability for the entire sample (r = 0.86; 95% lower limit CI = 0.80) 6. Interrater/Intra-rater Reliability: Various Injuries of Lower Extremity: Excellent interrater reliability (r = 0.84)6 LEFS SCALE validity: Construct Validity: Various Lower Extremity Injuries: Excellent correlations between the LEFS scores and the SF-36 physical function subscale and physical component summary scores (r = 0.80; 95% lower limit CI=.73) and (r = 0.64; 95% lower limit CI = 0.54), Poor correlation between the LEFS scores and the SF-36 mental component summary scores (r = 0.30; 95% lower limit CI = 0.14)6 Another reference reported that, Lower Extremity Functional Scale may be an alternative to the Western Ontario and McMaster Universities Osteoarthritis Index physical function scale. I will not change the outcomes measurement (LEFS) for knee injuries, I personally, prefer LEFS scale for its ease way and quick appliance to the patient. More than one strong study support high evidence, validity and reliability of LEFS. : The LEFS has good measurement properties: test- retest reliability and cross-sectional construct validity and it could be an alternative to WOMAC-PF If I change the outcomes measure Ill use Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC).Comparison between WOMAC scale and LEFS scale showed approximate results of strong evidence according to validity and reliability to the both scales. According to Robertson, a study of IRS attendee in British Columbia in 1991 indicated that 48% of former students had come across sexual abuse, 32% of them refused to answer, only a few of them claimed that they did not experience any abuse (2006). Along with the last residential school closed at 1996, the last cluster of students left the school and the government undertook the responsibilities of IRS, however, what had happened in IRS were irreversible. The experiences undergone in IRS were detrimental to those school attendee. The majority of IRS students were enduring psychological problems: post-traumatic stress disorder (PTSD), residential school syndrome (RSS), and historic trauma (Robertson, 2006). Post-traumatic stress disorder Post-traumatic stress disorder (PTSD) was the most common diagnosis in former IRS students, stood for approximately 64%. Symptoms of PTSD is partially similar to RSS but PTSD would influence not only psychological aspect, but also physically, emotionally, physiologically, and spiritually torture a PTSD patient. According to Sochting, Corrado, et al, the majority of IRS students conformed to the symptoms of complex PTSD: impairment in regulating affective impulses, in particular, anger directed at both self and others, chronic self-destructive behaviors, such as self-mutilation, eating disorder, or substance abuse. Some of IRS survivors also indicated that they had a chronic headache, heart problem, and arthritis (2007). Additionally, they would develop defense mechanisms to protect themselves not to experience the tragedy again, such as suppression, inability to express or acknowledge their feelings, and stopping mechanism. Suppression is a mechanism that would contribute to gap memo ries and patients would become apartness and inferior; and stopping mechanism is shutting off their feeling or bodily functions related to the experiences in IRS (Chansonneuve, 2005). These mechanisms affect PTSD patients in various ways, for example, their relationship with other: a female IRS survivor elaborated how she hurt her children because of lacking empathy after traumatized, she anticipated her children to be perfect, all the things had to be done in a particular way and time, which was the same way she was used to be taught. One of her children suffered from anorexia later which is a disorder that people is losing appetency to eat and drink (Grant, 1996). On the other hand, their apartness contributes to their inability to decide as they also did not have a chance to make decisions. Approximately all the IRS students cannot make alternatives except as one of the alternative is good for them straightly. Residential School Syndrome (RSS) The Residential School Syndrome (RSS) is the one of the consequence after the feeling of indigenous children had been tried to close off and maltreated. (Grant, 1996). According to Robertson, some expert claimed that RRS was one type of PTSD, but Charles Brasfield defined RSSs standard and recognized several differences between these two disorders. On the other hand, there were not many RRS patients were diagnosed RSS, 6.3% of former students were diagnosed in a sampling survey in British Columbia. However, the symptoms of RRS is severe. The symptoms of RSS are as follow: Addiction of drugs or alcohols at an early age and always with anger; depreciation on dominant cultural activities; undergone a panic IRS school experience or related to a person who used to be an IRS student; the attitude to IRS is passive, anxious, angry, and unassisted; Keep dreaming the lives in IRS and tendency to feel the scenario in IRS reappear again; Feel extremely dismayed when stimulate by something or someone can remind them of their memories in IRS (2006). Besides, the patients tendency to get angry easily causes much physical abuse, their arousal sometimes lead to family violence. After long-time basis, the violence would affect generation by generation. Their next generation may also suffer from RSS (Robertson, 2006). Historic Traumas Historic trauma (HT), is defined as spiritual imbalance and cumulative emotional psychological wounding over the lifespan and across generation. Besides, HT is suggested for indigenous people as they had experienced genocide through IRS (Robertson, 2006). According to Robertson, IRS students diagnosed with HT always with those symptoms: depression, self-destructive behavior, the tendency to suicide, anxiety, inferiority, wage, and lacking emotional intelligence. The historical trauma would deliver to the next generation which is a mechanism of HT as the trauma had been ingrained in the culture and peoples memories. And people in the next generation would also underlie the thought of being lessness. On the other hand, some might say that RRS is similar with HT. However, these two are focused on two different aspects: RRS is focused on the individuals psychological aspect while HT is focused on the how the cultures affect by the trauma and how people in the communities be affected (200 6). IRS students suffering more pain than other aboriginal people? Some may say other aboriginal people also experienced a tough time in reserve, IRS survivors did not experience more pain than other aboriginal people (Robertson, 2006). Virtually, some of former IRS students were living well without problems and aboriginal people in reserves were also undergone abuse and had the tendency to suicide. However, those were a rare part of people that did not experience maltreat or abuse in IRS and aboriginal people in the reserve did not suffer from the chronic stress (Elias et al, 2012). IRS survivors also lost their ability to learn and express feeling, while native people in reserve were not (Grant, 1996). Conclusion Indigenous people undergone a tragedy in IRS and these memories contributed to various problems on them, especially mental problems which cannot be erased and like a shadow following with their whole life. Although the IRS era had gone, we still can see the effect of IRS on generations. The majority of IRS survivors had diagnosed more than one psychological disorder, and most of them were alcoholic. Alcoholic parents may make their children feel shame while their children may also suffer from maltreatment. When the children grow up, they would know their culture was not being accepted in nowadays dominant culture and they would start drinking and suffer from historical traumas (Grant, 1996). Obviously, this is a vicious cycle but it is exactly what happening right now in the society. Fortunately, there is various way to heal with IRS survivors, such as their culture, language loss, and their mental health can be fixed but it does take a long time. This paper will examine the differences in competencies between nurses prepared at an associate-degree level versus a baccalaureate-degree level, in order to provide an evidenced-based understanding of the variation in the educational preparation of nurses. An associate-degree nursing program usually takes place over the course of a two year period, providing the nurse with the confidence in skills and knowledge to be placed in an entry-level position upon graduation. These types of programs focus on preparing nurses for care settings including community hospitals and long-term care facilitiesââ¬âand were traditionally designed to compensate for the nursing shortage. A bachelor-degree nursing program typically takes place over a period of four academic years; and is intended to prepare its graduates to practice nursing in leadership and management positions in a number of care settings (Creasia & Friberg, 2011, pp 25-27). A bachelor-degree program in nursing provides a further understand of not only the scientific and clinical nursing education, but also a more in-depth overview of specialized skills including: critical thinking, decision-making, communication, leadership, case management, and health promotion (The Impact of Educati on on Nursing Practice, 2012, para. 1). The American Association of Colleges of Nursing describes evidenced-based recommendations that explain that in order to ââ¬Å"respond to the demands of an evolving health care system and meet the changing needs of patients, nurses must achieve higher levels of education (Fact Sheet: Creating a More Highly Qualified Nursing Workforce, 2012, para. 5).â⬠The professional nurse holding a baccalaureate-level degree is prepared for a broader role in patient care with a higher understanding of holistic treatment, community health, clinical research, and nursing leadership and management. The added course work provided in a baccalaureate program is designed to prepare the nurse for a broader scope of practice and a better understand of issues that affect patients and their health care, including: cultural, economic, political, and social issues. ââ¬Å"Nurses with Bachelor of Science in Nursing (BSN) degrees are well-prepared to meet the demands placed on todayââ¬â¢s nurse (The Impa ct of Education on Nursing Practice, 2012, para 1).â⬠Patient outcome are the principal for continuing education. The level of educational preparation required by a baccalaureate-degree nurse allows for more equip decision-making in approach to patient care situations. According to the American Association of Colleges of Nursing, In the October 2012 edition of Medical Care researchers from the University of Pennsylvania found that surgical patients in Magnet hospitals had 14% lower odds of inpatient death within 30 days and 12% lower odds of failure-to-rescue compared with patients cared for in non-Magnet hospitals. The study authors conclude that these better outcomes were attributed in large part to investments in highly qualified and educated nurses, including a higher proportion of baccalaureate prepared nurses. (Fact Sheet: Creating a More Highly Qualified Nursing Workforce, 2012, para. 9) With ââ¬Å"Magnetâ⬠indicating a higher portion of baccalaureate nursing staff, it is determined that the nurse prepared at a bachelore-degree level is better prepared to approach a patient care situation with the capability of taking immediate action to provide better patient outcomes, than that of a nurse prepared at the associate-degree level. This goes to show that higher education has a strong impact on nursing practice. According to the American Association of Colleges of Nursing, a better educated nursing workforce will improve patient safety and enhance nursing care, decreasing mortality rateââ¬âas there is a significant correlation between nurses educated at a bachelor-degree level and lower patient death rates (Fact Sheet: Creating a More Highly Qualified Nursing Workforce, 2012). Through this research it is apparent that there is a significant connection between patient health care outcomes and the educational preparation of nurses. It is now established that the nurse prepared at a baccalaureate level is more readily capable of caring for critically ill patients; working in situations requiring critical thinking, leadership, and decision-making; and also taking roles in clinical aspects relevant to non-hospital settings. ââ¬Å"Researchers have identified improved patient safety and lower rates of patient morbidity and mortality; lower levels of medication errors and procedural violations; and fewer disciplinary actions for BSNs (Altmann, 2012, para 4).â⬠The need to continue education in nursing is on-going in order to continue to keep up with the changes in technology, advances in medicine, and to provide the best patient care. The Grand Canyon University Philosophy for nursing education explains: Baccalaureate nursing practice incorporates the roles of assessing, critical thinking, communicating, providing care, teaching, and leading. The caring professional approach includes the values of autonomy, altruism, human dignity, integrity and social justice with unconditional regard for all people. Nursing practice includes health promotion, disease prevention, early detection of health deviations, prompt and adequate treatment of the human response to acute and chronic illness, and compassionate care for those experiencing death. (Philosophy, n.d., para. 8) It is essential to maintain competency in nursing practice through the pursuit of continuing education. The difference between educational levels involves clinical competencies in a variety of settings, decision-making skills, leadership roles, job opportunities, and most importantly overall patient outcomes. IKEAââ¬â¢s brand Essay
Due to the recent uproar in German media, IKEAââ¬â¢s brand and image are being-à «rimshed, and I believe IKEA needs to respond to this situation immediately. IKEAââ¬â¢s supplier, Rangan Exports, haJviolated its supply contracts by using child labor for the production oflndian rugs, and such media exposure could lead to sizeable long-term problems such as disruptions in supply and sourcing, lost sales and profits, and reputation risk. I have applied 4 key decision-making criteria to the alternatives IKEA could employ to rectify this situation (Exhibit 1). Above all, since IKEA has been linked to various environmental/social issyes in the past, it should take a proactive stance on the issue of child labor by participating in the media. Without such action, IKEA runs the risk of hurting its image, potentially leading to a cut in its bottom line and slog growth opportunities (Exhibit 2). When looking at the first two alternatives, IKEA could deal with the issue of child labor through its own relationships with suppliers by forming a New Initiatives Steering Committee to manage the issue of child labor. On the other hand, IKEA shares a very important relationship with its suppliers and has many issues to manage on a day-to-day basis, like development and innovation (Exhibit 3). It is very likely that other issues, like child labor, could be overlooked from time to time. For instance, despite IKEAââ¬â¢s past initiative of adding an addendum to its supply contract, Rangan committed a breach of contract by utilizing child labor. For the third and fourth alternatives, IKEA could also step back and allow Rugmark to monitor the use of child labor on its behalf. By joining hands with Rugmark, IKEA would be able to re-emphasize its spirit of responsibility and increase consumer confidence. In addition, Rugmark is an Indo-German organization and such an association could help boost their image and sales in Germany, which account for 30% of total sales. In all the alternatives stated previously, IKEA needs to determine if it should terminate its contract with Rangan or not. If IKEA keeps Rangan on board, it will eliminate supply disruptions and prevent a drop in sales. However, sourcing a new supplier will send a stronger message to other suppliers about the consequences of breach of contract and the division will avoid future costly media outbreaks. For the last alternative, IKEA could withdraw from the Indian rug industry, because it accounts for a tiny part ofiKEAââ¬â¢s turnover. However, Indian rugs are complementary products to other IKEA product lines and discontinuing them would take away from the full line of products. Also, by retracting from the Indian carpet industry and not dealing with the issue of child labor, IKEAââ¬â¢s image as a socially responsible company could be tainted.
